Margaret Leonard, President Margaret is nationally known for her work in case management, care coordination,
transitions of care, certification and public policy. She has served on the National Quality Forum's Care Coordination Committee,
CMS Care Transitions Project - Technical Expert Panel, the Oregon Evidence-Based Practice Center - Technical Expert Panel
- Comparative Effectiveness of Case Management for Adults with Medical Illness and Complex Care Needs; and co-chaired NYS
DOH Case Management Work Group, She chairs the public policy committees of the national Case Management Society of America
(CMSA) and the National Transitions of Care Collaborative (NTOCC). Margaret collaborated to procure and implement a 3 year,
$3 million grant from NYS-Department of Health for a Chronic Illness Demonstration Program for integrated care coordination.
She developed the first American Nurse Credentialing Center (ANCC) certification exam for nurse case managers and co-authored
the ANCC Nursing Case Management Review and Resource Manual. Margaret is a past-president of the National CMSA and their Case
Manager of the Year in 2006. She is a national speaker and published author on case management and sits on two editorial boards.
She is a member of the New York State Board on Nursing and is adjunct faculty at the College of New Rochelle, School of Nursing.

Charlene Schlude, Vice President Charlene is currently the Director of Care Coordination at the Capital District Physicians' Health Plan responsible
for the oversight of the Case and Disease Management Programs, including the embedded Case Management Program offered to Patient
Centered Medical Home practices. She has over 14 years experience in the department of Medical Affairs at CDPHP and has been
influential in the development and expansion of its complex case management program. Previously she enjoyed a 14 year career
at the Albany Medical Center, in the Pediatric Intensive Care Unit and Post Anesthesia Recovery Room. Charlene is the current Chairman of the Healthy Capital District Initiative, which is a not-for-profit board dedicated
to improving public health and access to care for uninsured populations in the Capital District. She has participated in local
and national presentations on the role of embedded case management. She is also the current Vice President of the NY Capital
Region Chapter of CMSA and is a certified in case management through The Commission for Case Management Certification (CCMC).
Irene Grey, Treasurer Irene has extensive background in acute care hospital administration, with an emphasis on performance improvement,
regulatory, risk management activities, case management, social work and utilization management. More recently, directed
inpatient utilization management activities for CDPHP. Abundant knowledge of federal and state hospital regulations,
regulatory standards and codes, Joint Commission preparation and accreditation, and CMS Core Measure Project, NCQA standards
as well as all aspects of performance improvement. Proven record in managing professional and ancillary staff with an

Cashelle has been employed in the specialty field of occupational health since
1999 in various facilities both non-profit and for-profit companies. Cashelle joined membership in numerous nursing
organizations within New York Capital Region and Southern Vermont. She has developed proficiency in providing work-site health
and wellness, work safety, ergonomics and injury prevention programs. While enrolled as a graduate student, her
practicum project focused on establishing a Healthy Aging Workforce. Cashelle
received her CCM certification in 2003 from Certified Case Management certification. She is currently employed

Dawn Cooke, RN, CCM Dawn has been involved in the Nursing field since 1976, working at Samaritan Hospital in Pediatrics and OB/GYN.
In 1989 she became involved in Occupational Health Nursing while initially working as the Nursing Supervisor and later the
Coordinator of Occupational Health Services at Capital Healthcare Associates in Troy, NY. In 2002 she began her career
in Case Management, working with cases involving Workers Compensation, Short and Long term disability and performing second
level FMLA reviews at Reed Group. Dawn received her certification
as a Case Manager in 2005 and as of July 2012, she joined the staff

Sheilah McGlone RN, CCM, BC Sheilah is currently the Senior Director of the Clinical Department at Hudson Health
Plan who oversees all case management programs. She is also a Co-Director of the Westchester Cares Action Program, an award
winning integrated care management program that is part of a New York State Department of Health Chronic Illness Demonstration
Project. She has worked for Hudson for 16 years and has over 20 years of case management experience that encompasses commercial
and government funded environments. Prior to stepping into a corporate role, Sheilah practiced as a Registered Nurse
in various hospitals acute care settings, specializing in critical care. She is also a certified Community HIV/AIDS Educator.
She created and implemented a unique Pediatric Lead Case Management Program which won the Clinical Innovation Award at the
2004 New York State Nurses Association Convention. Sheilah has participated in several presentations pertaining to case
management on a local and national level and has held various leadership
positions within the Hudson Valley Chapter of CMSA that include Chapter President, Education Chair, and Treasurer. She currently
remains active as a board member of the Hudson Valley Chapter. She was also the recipient of the Hudson Valley Chapter's first
Case Manager of the Year Award. Sheilah was also active with the New York State Case Management Work Group, has participated
in the beta testing of the Integrated Case Management Model training course and the model's "Train the Trainer"

Linda Baker RN-BC ,CPC Linda has practiced as a Registered Nurse for 28 years in various OB and behavioral health acute care settings and
has over 8 years of case management and utilization review experience that encompasses both commercial and governmental funded
environments. She is currently the lead Perinatal Nurse Case Manager at Hudson Health Plan. In this role she oversees the
case management activities within the plan's "Mommy & Me" Program. Linda utilizes her skills in assessing perinatal
patients for High Risk pregnancies, psychosocial issues including field case management, assisting the members in obtaining
housing, baby items, childbirth and breast feeding classes. Educating members regarding: current health issues, breast feeding,
vaginal deliveries and promoting healthy birth outcomes. Linda's has excellent clinical assessment skills which enables her
to provide the best Case Management to her perinatal patients. Linda works very closely with Hudson Health Plans behavioral
health provider to assure her members needs are met. Her previous work experience includes positions as a Charge Nurse
on a Mental Health Unit, Senior Quality Improvement Specialist for a behavioral health unit and Senior Health Care Investigator
for Medicare and staff nurse for OB/GYN practice. Throughout her extensive career she has earned certifications in the
following areas; Clinical Case Management; Adult Mental Health Nursing; HIV/AIDS counselor; Group Therapy Leader; Legal Nurse
Consulting and Physician Coder. Linda is an active participant in the NYS DOH Case Management Task Force, and serves
on the board of the Hudson Valley Perinatal Forum, and a member of
the Perinatal Mood Disorder Task force for Orange County. Linda is currently serving on the Content Expert Panel for the American
Nurses Credentialing Center Case Management exam. Linda is currently the treasurer for the Lower Hudson Valley Chapter Case
Management Society of America. Linda also attended the Public Policy Summit in Washington DC in 2010 to promote Case Management
in the Health Care System.
Christine Robinson, RN, CCM Since 2007, Christine has been employed at The Reed Group as a RN CCM handing Short Term Disability cases.
Prior to that, she worked in the Long Term Care setting at Hoosick Falls Health Center as a RN-C in Gerontology.
Christine's experience also includes working as a RN at the NYS Department of Medicaid, House Supervisor at Uncle Sam's
House managing 3 Transitional houses for Homeless Veteran's, and the Eddy and the VNA doing home care and co-managed a Dairy
Farm in White Creek, NY for 16 years. Affiliations:

Margaret Messier, RN, CCM Present Position: Retired Margaret's
most recent position was that of a Clinical Education Consultant for McKesson where she provided on-site and Web-based InterQual
clinical product education and software implementation assistance for managed care organizations and hospitals throughout
the United States. Marge also has over 15 years of managed health care experience holding various positions with MVP
Health Care in Schenectady, NY. Marge was the MVP Health Services Training Manager responsible for training programs
for the Utilization Management and Member Services Departments and was in charge of the Utilization Management and Medical
Director Quality Control programs. She also worked as a Program Manager where she was a member of a team responsible
for the merging of medical policies for the newly combined companies of MVP Health Care and Preferred Care. Additionally,
Marge worked as a Transplant Case Manager, Retrospective Review Supervisor and Utilization Management Nurse. Other experience
includes 20 years of clinical nursing encompassing Medical Surgical, Pediatrics, Critical and Progressive Cardiac Care, Labor/Delivery/Post
Partum, Orthopedic and Urgent Care. Affiliations:
